New Doctor of Medicine Develops New Method for Osteoarthritis Treatment



Located in the Lt III Postgraduate meeting room, a Doctoral Promotion exam was taking place with promovendus candidate dr. Cokorda Gde Oka Dharmayuda, Sp.OT(K) with the dissertation title "Mesenchymal Stem Cell Derivative Sekretome Causes Lower Levels of Matrix Metalloproteinase-13, AgREkanase-2, and Interleukin-1β Compared to Administration of Hyaluronic Acid in Rabbit Knee Osteoarthritis."


Osteoarthritis (OA) is the most common form of arthritis, causing pain and disability that can affect quality of life. OA is caused by joint cartilage damage due to an imbalance between the damage process and the cartilage regeneration process. Previous treatment of OA used intra-articular injections of hyaluronic acid, but several studies have shown unsatisfactory clinical results. Later, secretome therapy was developed for mesenchymal stem cell derivatives which are thought to have anti-inflammatory, anti-catabolic, and anti-apoptotic effects so that regeneration processes can occur. This study aims to determine and compare the anti-inflammatory and anti-catabolic effects of mesenchymal stem cell derivative sectome compared to administration of hyaluronic acid as the gold standard of OA treatment.


The results obtained from this study showed that the levels of MMP-13, Agrekanase 2, and IL-1β in rabbit knee osteoarthritis were significantly lower after administration of mesenchymal stem cell derivative secretome compared to administration of hyaluronic acid. This result is statistically significant based on the Independent T-Test with a p< 0.05


The development of a new science (novelty) from this research is that administration of secretome can reduce levels of MMP-13, agrekanase-2, and IL-1β better than administration of hyaluronic acid in rabbit osteoarthritis. To the best of the researchers' knowledge, there have been no studies comparing AH injection with mesenchymal stem cell-derived secretomes in rabbit knee osteoarthritis, especially those that discuss proteolytic enzymes and inflammatory mediators.


The implication of the results of this study is that the administration of mesenchymal stem cell derivative secretome in OA of the knee can cause a decrease in the levels of pro-inflammatory cytokines and proteinase enzymes lower than the administration of hyaluronic acid in rabbit knee osteoarthritis. This study is able to add to the justification of scientific evidence to support phase 1 clinical trials regarding mesenchymal stem cell-derived secretomes as a breakthrough therapy for knee OA.
