Ultraviolet light triggers the production of reactive oxygen species (ROS) causing accumulation of oxidative damage and oxidative products which are markers of oxidative stress and one of the targets is the skin. Dermis fibroblasts as the predominant cell type in the dermis, play a role in producing collagen and actively forming the extracellular matrix. Fibroblasts play an important role in the photoaging process, especially after the skin is exposed to UV light. This study aims to prove the protective effect and collagen stimulatory effect of a combination of hyaluronic acid and calcium hydroxyapatite with lower c-Jun and matrix metallo-proteinase expression, as well as transforming growth factor-β and higher type 1 collagen density compared to intradermal injection of single hyaluronic acid.
This study used a randomized post-test only control group design. A total of 30 male Wistar rats were randomly divided into 3 groups, namely the normal group (P0), the control group (P1) and the treatment group (P2). The control group was injected with hyaluronic acid intradermally and then exposed to UVB 3x/week for 4 weeks with a total dose of 840 mJ/cm2. The treatment group was injected with a combination of hyaluronic acid and calcium hydroxyapatite intradermally and then exposed to UVB 3x/week for 4 weeks with a total dose of 840 mJ/cm2. While the normal group was not given any treatment. The comparability test between groups used the One Way Anova Test followed by a Post-Hoc test using the Least Significant Difference (LSD) Test. The results showed that the combination of calcium hydroxyapatite and hyaluronic acid caused c-Jun expression to be significantly lower than the normal group and the hyaluronic acid administration group (P2-P0 p=0.001; P2-P1 p< 0.001), MMP-1 expression was significantly lower than the normal group and the hyaluronic acid administration group (P2-P0 p< 0.001; P2-P1 p< 0.001), TGF-β expression was significantly higher than the normal group and the hyaluronic acid administration group (P2-P0 p< 0.001; P2-P1 p< 0.001) and type 1 collagen density was significantly higher than the normal group and the hyaluronic acid administration group (P2-P0 p< 0.001; P2-P1 p< 0.001).
Intradermal injection of a combination of hyaluronic acid and calcium hydroxyapatite causes lower c-Jun and MMP-1 expression, and higher TGF-β expression and type 1 collagen density compared to single hyaluronic acid administration in Wistar rats exposed to UVB.
