Held at the Postgraduate Meeting Room (3rd Floor, Postgraduate Building, Udayana University, Denpasar), the Doctoral Program in Biomedical Sciences at the Faculty of Medicine, Udayana University, conducted an open doctoral defense for candidate Dr. Stella Kawilarang, M.Biomed., Sp.OG. Her dissertation was titled "The Effect of Umbilical Cord Mesenchymal Stem Cell Secretome Administration on Ovarian Weight, Follicle Count, Anti-Müllerian Hormone Levels, and Pregnancy Rates in Wistar Rats with Premature Ovarian Failure." Premature Ovarian Failure (POF) is a condition of early menopause occurring in women of reproductive age (under 40 years old). The incidence of POF is estimated at approximately 1 in 100 women; however, rates have risen over the last decade, linked to an increase in gynecological cancers requiring chemotherapy, which can ultimately damage reproductive function. To date, there is no definitive therapy to restore reproductive function in POF cases. While stem cell administration is a management option, concerns regarding potential side effects and high costs have created a need for alternative regenerative therapies—such as secretome, a stem cell-based therapy that offers capabilities similar to stem cells. This study aims to demonstrate that umbilical cord mesenchymal stem cell secretome can improve ovarian function in POF cases following chemotherapy, evaluating parameters ranging from hormonal function and morphology to fertility outcomes, specifically pregnancy and parity.
The study employed a randomized post-test-only control group design. Eighteen Wistar rats were allocated into three groups: a control group, a negative control group (administered cyclophosphamide), and a treatment group (administered cyclophosphamide and secretome). Measurements of ovarian weight, follicle count, and AMH levels were performed by independent, blinded assessors; data analysis utilized One-Way ANOVA and Mann-Whitney U tests with a significance level of p< 0.05. A follow-up study employed the same grouping design but included mating with male rats to assess pregnancy rates and parity.
Results from the first study showed a significant and consistent increase across all variables—both hormonal and ovarian morphological (p< 0.05)—in the test group compared to the negative control group. Consequently, a second follow-up study was conducted to assess fertility function through mating with male rats; this also yielded significant increases in both pregnancy rates and parity (p< 0.05). However, when comparing the test group results with the normal control group, significant differences (p< 0.05) remained across all assessed parameters.
The study concludes that the administration of umbilical cord mesenchymal secretome can enhance or restore ovarian function parameters—ranging from hormonal and morphological aspects to fertility outcomes such as pregnancy rates and parity—in cases of iatrogenic premature ovarian insufficiency (POI) following chemotherapy. However, the results did not fully match those of the group with normal ovarian function. These findings provide a theoretical basis for further research, particularly benefiting women of reproductive age who are cancer survivors and wish to preserve their fertility following chemotherapy.
