Located in the Prof. dr. I Goesti Ngoerah Gde Ngoerah, there has been a Doctoral Promotion exam which was carried out in a hybrid manner on behalf of dr. I Gusti Ngurah Wien Aryana, Sp.OT (K) with the title "The Administration of Recombinant Platelet Derived Growth Factor-BB to Tendon Autograft Causes Vascular Endothelial Growth Factor Expression and Type I/III Collagen Ratio and Stronger Tensile Strength After Ligament Rupture Reconstruction The Rabbit Anterior Cruciatum."

Anterior cruciate ligament (LCA) rupture is one of the most common ligament ruptures. LCA rupture is treated operatively by means of ligament reconstruction. Ligament reconstruction aims to restore the function of the ruptured ligament using a tendon graft. However, failure can occur because the tendon graft does not undergo the process of angiogenesis and ligamentization. To reduce the risk of this failure, tissue engineering was developed using a tendon autograft and growth factor.

The main results obtained from this study were that the administration of tendon grafts had an effect on the angiogenesis process after the administration of PDGF after the reconstruction of the anterior cruciate ligament rupture in rabbits through the role of VEGF, type I/III collagen ratio and tensile strength. VEGF expression in tendon autografts treated with rhPDGF-BB after reconstruction of anterior cruciate ligament rupture in rabbits was higher than in tendon autografts not treated with rhPDGF-BB. Type I/III collagen ratio in tendon autograft treated with rhPDGF-BB after reconstruction of anterior cruciate ligament rupture in rabbits was higher than in tendon autograft not treated with rhPDGF-BB. Tensile strength in tendon autograft treated with rhPDGF-BB after reconstruction of anterior cruciate ligament rupture in rabbits was stronger than tendon autograft not treated with rhPDGF-BB.

The novelty of this research is to provide an understanding of the administration of PDGF-BB modifying the effect of angiogenesis by inducing an increase in VEGF, thereby increasing the ratio of collagen type I to type III, and producing better tensile strength. Because based on the theory PDGF-BB has chemotactic and mitogenic properties that function as factors that can improve wound healing, and is also proangiogenic, thus helping up-regulating VEGF to stimulate the growth of new capillary blood vessels.
The implication of this study is that PDGF-BB can be an alternative adjuvant therapy to improve biomechanical, clinical, and functional outcomes in patients with anterior cruciate ligament rupture who undergo ligament reconstruction procedures with tendon autografts. Further research in the form of further experimental studies in human-like animals is needed to investigate the possible side effects and efficacy in human-like animals, which will then be re-evaluated prior to clinical trials on human subjects.
