Bali Endocrine Update XXIII: The New Era of Metabolic Health: Beyond Comprehensive Endocrine Care.

The Faculty of Medicine of Udayana University, in collaboration with the Bali Branch of the Indonesian Endocrinology Society (PERKENI), held the Bali Endocrine Update (BEU) XXIII scientific meeting with the main theme "The New Era of Metabolic Health: Beyond Comprehensive Endocrine Care." The three-day event series began with a workshop on Friday, April 24, 2026, held on the 4th floor of the Angsoka Building, Prof. Ngoerah General Hospital. The second day featured a symposium on Saturday and Sunday, April 25-26, 2026, at the Harris Hotel and Conventions Denpasar, Bali. The event also included a scientific competition in the form of oral paper presentations and case reports, conducted online. The purpose of this symposium is to provide an update on the management of increasingly complex metabolic and endocrine diseases, along with the emergence of the latest therapeutic innovations in 2026.

The event began with remarks from the Chair of the Committee, Dr. dr. Wira Gotera, Sp.PD-KEMD, FINASIM., followed by remarks by the Dean of the Faculty of Medicine, Udayana University, Prof. Dr. dr. I Gede Eka Wiratnaya, Sp.OT., Subsp.Onk.Ort.R(K).

This symposium featured fifteen (15) highly competent speakers in their fields:

1. Dr. dr. Wira Gotera, SpPD, K-EMD, FINASIM, with topics on "Cardiorenal Protection in Diabetes: The Role of Eplerenone," "Elevate your Glucose Control: From Initiation to Early Intensification Through Fixed Dose Combination Therapy," and "Optimizing Glucose Control Outcomes with a Fixed Ratio Combination of Sitagliptin and Metformin." 2. dr. Andreas Nuho Fernandez Lewai, SpPD, K-EMD with material, The role of Biosimilar Insulin Glargine in Expanding Access to Basal Analog Therapy (product) Sansulin Log G.

3. dr. Gede Wira Mahadita, M.Biomed, SpPD., K-GH., FINASIM with material on Overcoming Hyperkalemia in DKD: Novel Potassium Binder to Rapidly Correct and Sustain Cardiorenal Protection.

4.Dr. Dr. Made Ratna Saraswati, SpPD., K-EMD., MARS., FINASIM, with material Addressing Unmet Needs in T2D and Chronic Weight Management with Tirzepatide.

5.Dr. Dr. I Made Pande Dwipayana, SpPD, K-EMD, FINASIM, with material on Obesity and Dual Agonists: Comprehensive Weight Management without Borders and Timeless or Time's Up? Rethinking Sulfonylureas in the Modern Diabetes Algorithm.

6. Dr. Dewi Catur Wulandari, SpPD, K-EMD, FINASIM, with the topic "Combining Oral Antidiabetic Drugs: Focus on Patient-Centered Strategies."

7. Dr. I Made Siswadi Semadi, M.Biomed, SpPD, K-EMD, FINASIM, with the topic "The Pre-Diabetes Puzzle: Unraveling Risk Factors and Treatment Pathways."

8. Dr. dr. Libriansyah, SpPD, K-EMD, FINASIM, AIFO-K, MM, CIPA, with the topic "Cardio Renal Metabolic Protection in MASLD: The Role of SGLT2 Inhibitors" and "Comprehensive Cardiometabolic Management in Type 2 Diabetes: The New Standard of Care: Integrated Vascular Protection in Type 2 Diabetes."

9. Dr. dr. Tjok Gde Dalem Pemayun, SpPD, K-EMD, FINASIM, with the topic "What You Need to Know Beyond the Numbers: Understanding Obesity as a Chronic Disease."

10. Dr. dr. Made Ratna Saraswati, SpPD, K-EMD, MARS, FINASIM, with the topic "GLP-1 RA: A Proven Standard for Evidence-Based Obesity Management" and "Optimizing Insulin Glargine U-300 in Non-Critically Ill Patients: A Strategy for Better Glycemic Control."

11. Dr. dr. I Made Pande Dwipayana, SpPD, K-EMD, FINASIM, with the topic "Optimizing Insulin Therapy: From Basal Initiation to Rapid Intensification."

12. Dr. Dewi Catur Wulandari, SpPD, K-EMD, FINASIM, with the topic "From Glucose-centric to Organ-protective Strategies in Type 2 Diabetes Management."

13. dr. I Made Siswadi Semadi, M.Biomed, SpPD, K-EMD, FINASIM, presented on "Maximizing Postprandial Glycemic Control with Insulin Glulisine: Strategies and Benefits."

14. Dr. Ida Bagus Aditya Nugraha, SpPD, M.Biomed, FINASIM, presented on "Spotting the Storm: A Practical Clinical Approach to Recognizing and Managing Thyroid Storm."

15. Prof. Dr. dr. A.A. Gede Budhiarta, SpPD, K-EMD, presented on "Mastering the Clinical Diagnosis of Myxedema Coma in Daily Practice."

The topics covered a broad spectrum of endocrinology and metabolism, including Cardiometabolic and Renal Protection in Type 2 Diabetes Patients, Obesity Management, Optimizing the Use of Basal Analog Insulin and Appropriate Insulin Intensification, and Endocrine Emergencies.

Through the 23rd Bali Endocrine Update, it is hoped that medical professionals will deepen their clinical skills and adopt the latest standards of care to provide comprehensive and innovative healthcare for patients with metabolic disorders in Indonesia.

The presentations will be followed by an interactive and in-depth discussion session. The symposium is designed specifically for medical students, general practitioners, interns, and internal medicine specialists.
