Doctor of Biomedical Sciences Studies a Natural-Based Combination Therapy Approach as a Safe and Effective Alternative for Postmenopausal Knee Osteoarthritis
Held in the Dr. A.A. Made Djelantik Meeting Room, Fourth Floor, Faculty of Medicine Building, Jl. PB Sudirman, Denpasar, Bali, the Doctor of Medical Sciences Study Program, Faculty of Medicine, Udayana University, held an open doctoral promotion defense with candidate Dr. Putu Kermawan, Sp.OT, M.Biomed (IKD), AIFO-K, FICS., presenting a dissertation entitled "The Effect of a Combination of Shrimp Shell Extract, Soybean Extract, and Paracetamol on Levels of Soluble IL-6 Receptor, IL-10, and Cartilage Oligomeric Matrix Protein in Joint Fluid, as well as VAS Values and Scores from Western Ontario and McMaster Universities in Postmenopausal Women with Knee Osteoarthritis." (January 23, 2026)
Osteoarthritis (OA) of the knee is a degenerative disease that causes pain and disability, especially in postmenopausal women. OA treatment typically uses NSAIDs, which have gastrointestinal side effects. An alternative therapy for knee OA using a combination of shrimp shell and soybean extract (containing glucosamine, chondroitin, chitosan, and phytoestrogens) and paracetamol is expected to be safer and more effective. This study evaluated the effect of this combination on sIL-6R, IL-10, and COMP levels in joint fluid, as well as VAS and WOMAC scores in postmenopausal patients with knee OA.
A double-blind, randomized, controlled clinical trial was conducted in postmenopausal patients with knee OA. Subjects were divided into two groups: an intervention group receiving a combination of shrimp shell and soybean extract and paracetamol, and a control group receiving paracetamol and placebo. Measurements of sIL-6R, IL-10, and COMP levels in joint fluid were performed before and after treatment for 2 weeks. Joint pain and function were evaluated using the VAS and WOMAC scales before and after treatment for 2 weeks.
The results showed that sIL-6R, COMP, and WOMAC levels in the intervention group were significantly lower than those in the control group (p< 0.01 for sIL-6R and COMP; p=0.01 for WOMAC). IL-10 levels were also increased in both groups, with a more significant increase in the control group (p< 0.01). The VAS scores in the intervention group were lower, but not significantly lower, than those in the control group (p=0.78).
The combination of shrimp shell extract, soybeans, and paracetamol has been shown to be effective in reducing inflammation and pain, as well as improving joint function. The combination of shrimp shell extract, soybeans, and paracetamol has the potential to be a safe and effective alternative therapy for postmenopausal patients with knee OA. Further, longer-term studies are needed to further validate these results.
