Early Breastfeeding Initiation (IMD) is an important factor in reducing neonatal mortality. With strong policy support and efforts to increase the capacity of health workers, the practice of IMD can be one of the pillars in efforts to improve maternal and child health. However, the coverage of IMD in Indonesia, especially in Gianyar Regency, is still low. The knowledge and attitude of midwives' independent practice towards IMD are the determining factors for the success of this program.
This study aims to test the short training model for IMD management and counseling (12 hours) from the basic breastfeeding management and counseling module in health facilities by CPHI Udayana University and GIAN to improve midwives' knowledge and attitudes in implementing early breastfeeding initiation in Gianyar Regency.
This study design used a combination method with an explanatory sequential design approach consisting of 2 stages. The first stage of the quantitative study involved a total sample of 91 respondents from the Independent Midwife Practice in Gianyar Regency who were given a short training intervention for IMD management and counseling with a one group pretest-posttest design. Midwives' knowledge and attitudes were measured before and after the training intervention. Quantitative research used a paired t-test for normally distributed data and a Wilcoxon test for non-normally distributed data. Multivariate data analysis was performed using multiple linear regression. The second stage of the qualitative study involved 10 PMB informants with a purposive sampling technique to explore understanding of the implementation of IMD and perceptions of the short training model for IMD management and counseling.
The results of the quantitative study showed that short training increased knowledge scores by 5.97 (20.6%) and attitude scores by 5.16 (0.52%) after training intervention (p-value = 0.001). The Wilcoxon Test showed a significant increase (p < 0.001) in midwives' knowledge scores after participating in the training. The average knowledge score increased from pre-test by 29.01 to 34.99 (SD: 2.72) after training (post-test). The paired t-test also showed a significant increase (p < 0.001) in participants' attitude scores. The average attitude score increased from 99.48 before training to 104.61 (SD: 7.81) after training. Multiple linear regression analysis showed no significant relationship between age, education, and length of service with knowledge scores (p = 0.3115> 0.05). Only the education variable had a significant effect on attitude scores (p = 0.015). Negative regression coefficient (-2.419).
It can be concluded that the short training model for IMD management and counseling is effective in improving the knowledge and attitude of midwives' independent practice in Gianyar Regency. The training intervention not only provides cognitive information, but is also able to change the perception and attitude of midwives to be more supportive of IMD practice. The resulting short training model can be used as a reference for the development of similar training programs in other areas.
