Doctor of Medicine Develops Educational Model for Tourism Nursing Services


Held in the Postgraduate Hall on the third floor of the Postgraduate Building, Udayana University, the Doctor of Medicine Study Program, Faculty of Medicine, Udayana University, held an open doctoral promotion examination with candidate Ns. Kadek Eka Swedarma, S.Kep., M.Kes., entitled "Development of an Educational Model for Tourism Nursing Services for Nurses in Bali." (July 29, 2025)


Tourism is a strategic sector for economic development, with Bali as a major destination experiencing a significant increase in tourist visits. However, the high mobility of tourists increases the risk of health problems such as infectious diseases, environmental adaptation disorders, and physical trauma. Managing these conditions requires responsive and professional healthcare services, in which nurses play a central role. Unfortunately, many nurses in tourist destinations still lack adequate competency in tourism nursing services, particularly in aspects of cross-cultural communication, self-efficacy, and emergency preparedness.


This study aims to develop a theory-based educational model to improve nurse competency in the context of tourism nursing, namely through the Travel-CARE model, which integrates Transcultural Nursing Theory and Social Cognitive Theory. The study used a mixed methods approach with a sequential explanatory design, spanning three stages: quantitative study, qualitative exploration, and development and validation of the educational model. Quantitative data involved 116 nurses in four tourism areas in Bali, while qualitative data were collected through in-depth interviews with 11 participants, including nurses and nursing managers.


Quantitative results indicate that self-efficacy and culturally sensitive knowledge are significant factors influencing the quality of tourism nursing services. Qualitative findings support this by identifying the need for training, team support, standard operating procedures (SOPs), and multilingual communication facilities as important factors in supporting nurse readiness. Limited resources and professional networks were key barriers. The integration of these two findings resulted in the Travel-CARE Educational Model, consisting of four components: Cultural Sensitivity, Awareness, Readiness, and Empowerment, which was validated as appropriate by experts without revision.


This model was developed contextually, holistically, and practically for widespread use in travel nursing training. With a strong theoretical basis and empirical data support, the Travel-CARE model is expected to improve nurse competency, support the development of a professional travel nursing service system, and strengthen Bali's image and competitiveness as a healthy and safe global tourism destination. Adoption of this model also has the potential to encourage new policies in nursing education, recognition of specializations, and the formation of a community of practice in the field of travel health.
