Development of an Integration Model to Improve Knowledge and Perception of Health Workers in Screening and Management of Mental Disorders in Community Health Centers

Chronic non-communicable diseases are increasing throughout the world and in Indonesia, which affects mental health. However, screening and management of mental disorders have not been optimally integrated in community health centers, especially for high-risk groups, because integration guidelines are not yet available. This study aims to design and test a model and guidebook for integrating screening and management of mental disorders that are suitable for Community Health Centers in Bali.
The study was conducted from March 2023 to April 2024, with an Exploratory Sequential Mixed Method design (qualitative and quantitative approaches), consisting of: (1) Qualitative study with in-depth interviews (WM) and Focus Group Discussions (FGD). Qualitative data analysis with thematic methods (NVIVO-14); (2) Interim phase: compiling a guidebook for the integration of screening and management services for mental disorders, adopting the Doherty integration model, equipped with a screening and management flow for mental disorders, according to the WHO mhGAP guidelines combined with family medicine. (3) Quantitative study with intervention training in the use of guidelines with a pre-post test non-randomized controlled trial design for health workers at health centers. This study found that the obstacles in screening and management of mental disorders in health centers were: lack of human resources, facilities and infrastructure, medicines, no integration guidebook and lack of funding. The integration model that is considered feasible at this time is a fully integrated service which in this study is stated in the form of an EXAMED-FMC model guide. The results of the guideline trial showed that the average knowledge value in the intervention group increased significantly compared to the control. In the perception aspect, only the perception of management increased significantly in the intervention group (64.2%). There was no significant difference in motivation, experience and number of screenings due to the lack of policy support at the district level.
This study concludes that training in the use of the 'EXAMED-FMC' guideline can improve the knowledge and perception of Puskesmas HR, but has not been able to improve other variables. Policy support is needed to maximize the use of this guideline in the future
