Doctor of Medical Sciences Develops Holistic Model to Improve Quality of Life of People with HIV

Located in the Meeting Room of dr. A.A. Made Djelantik,, 4th Floor of the Faculty of Medicine Building, Udayana University, the Doctor of Medical Sciences Study Program, Faculty of Medicine, Udayana University held an open Doctoral Promotion examination with the candidate for promovenda Ns. Gusti Ayu Krisma Yuntari, S.Kep., M.Kes., with the dissertation title "Implementation of HOPE Intervention in Improving the Quality of Life of People with HIV (PLHIV)". (6/25/2025)

HIV/AIDS not only causes problems with physical conditions, but also has an impact on social and psychological conditions. This certainly affects the quality of life of PLHIV. Quality of life is an important indicator to determine the success of a treatment or intervention. Various interventions have been developed to improve the quality of life of PLHIV. This study developed an intervention using the HOPE intervention model (Health Education, PLHIV Group Support, and Phone Evaluation). The purpose of this study was to prove that the development of the HOPE intervention model can improve the quality of life of PLHIV in the physical, psychological, level of independence, social relationships, environment and spiritual dimensions.

This study used a true experimental design with the selected research design model being a randomized control trial. This study used a sample size of 25 HIV patients in each group at the Sanjiwani Gianyar Hospital in October-December 2024. This research used a measuring instrument in the form of the WHOQOL HIV BREF Questionnaire to assess quality of life.

This study shows that the quality of life of PLHIV in the intervention group increased compared to the control group. Characteristics of gender, occupation, education, distance from home to health care facilities, age and BMI did not provide significant differences (p>0.05). The HOPE Intervention Model has been shown to significantly improve the quality of life in the physical, psychological, level of independence, social relationships, environment and spiritual dimensions of PLHIV (p< 0.05). The HOPE Intervention Model provides the largest effect size in the physical domain of 0.78 and overall provides an effect size of 0.68.

The HOPE model developed with ecological theory, Social Determinants of Health theory and resilience theory has been proven to improve the quality of life of HIV patients in the physical, spiritual, independence level, psychological, social relationships and environment domains. The uniqueness of the HOPE intervention model lies in the interventions provided at various levels, namely the individual level by providing education, the community level by providing group support interventions and the health system level by providing phone evaluations. Through this model, more holistic interventions can be provided so that they can increase the involvement of PLHIV in care, reduce the negative impact of stigma in society and improve the quality of life.
